Bishop Gorman is offering Sports Physicals to all our students on Wednesday, May 21 from 5-7PM in the gym.  Every student must complete the main gym portion before being seen by medical staff.  The cost is $25 dollars payable on the night of the event. This is a cashless payment event there will be QR CODES posted to pay.
 
Instructions:
  1. Print and bring the “Sports Physical” packet (see Orange button at the right). Please complete the “Pre-participation history form” before you start the process in the main gym.
  2. In the Main Gym: Follow the traffic flow completing each step of the physical process.
  3. Proceed to the Aux Gym: Medical Staff will exam you. Complete and Sign both the “Pre-participation physical evaluation form” and the “Medical eligibility form”.
  4. Before leaving the Aux Gym: See Nurse Young to turn in your “Pre-Participation History Form” and “Pre-Participation Evaluation Form”.
  5. The “Medical Eligibility Form” is to be inserted into the 2025-2026 physical packet.  
  6. You are now ready to Register for all sports in the 2025-2026 school year. Complete the 2025-2026 Physical Packet (see Orange button at right) and register for all sports you are interested in on Aktivate. Must go to registermyathlete.com and register.

Register with Aktivate

All students attending Bishop Gorman MUST complete a Sports Physical Packet with a current physical exam dated after May 21, 2025.  This includes all freshman and transfer students.

The Sports Physical Packet is also available on Aktivate (formerly Register My Athlete). ). Pages 1 – 7 of the Sports Physical Packet must be signed, dated, and uploaded as a single PDF document into Aktivate to complete the student’s registration.

Students are NOT allowed to participate in any Bishop Gorman practices, workouts, or organized training sessions until their registration is complete for the intended sport.
 
Parents: If you are having trouble uploading your student’s completed Sports Physical Packet, the CamScanner App is a free tool to help scan the Packet into a PDF and upload it easily into Aktivate.  The App is free on the App Store or Google Play.
