You will need the following information to complete your application:

--copies of your most recent IRS federal income tax return 1040 or 1040A,
including all supporting tax schedules

--copies of your 2007 W-2 form

--supporting documentation for Social Security Income, Welfare,
Child Support, Food Stamps, Workers' Compensation, and TANE

--payment of your $50 nonrefundable application fee

Completed applications must be postmarked by January 31, 2008.

Financial Assistance notification will be mailed by March 10, 2008.
